The Effects on Pulmonary Rehabilitation Therapy of New Mechanical In-Exsuffla

Abstract
Objective: To identify the effectiveness on pulmonary function and safety of new mechanical in-exsufflator (MI-E) by comparison with usually used device in patients with restrictive lung disease, who need a pulmonary rehabilitation therapy.

Methods: Sixteen restrictive lung disease patients who had stroke or spinal cord injury were divided into two groups; experimental (n=8) groups and control (n=8) groups. Experimental group performed new MI-E device (Cough and Suction¨Þ) daily for 3 weeks while control group performed usually used MI-E device (Cough Assist¨Þ) with same procedure and schedule. The pulmonary function was evaluated by measuring percentage of predicted value of forced vital capacity (FVC), maximal inspiratory capacity (MIC), peak cough flow (PCF), maximal expiratory/inspiratory pressure (MEP/MIP). These data of pulmonary function before and after treatment were compared between two groups.

Results: The experimental group showed equivalent effect as the control group in improvement of pulmonary function. Also there is no serious adverse event in both groups. No adverse events were reported with two devices for study period. Questionnaire survey to therapists and patients showed that most disadvantage was the absence of automatic alteration of positive pressure and negative pressure in new device despite similar satisfaction.

Conclusion: The new MI-E device can be used as an effective therapeutic modality for the improvement of pulmonary function.
